- Cholesterol under 210 and HDL less than 5.0
- No history of cancer, diabetes, heart disease, etc.
- Maximum 2 moving violations past 3 years.
- No record of DUI or reckless driving.
- No use of tobacco products in past 3 years.
- No history of alcohol or substance abuse.
Preferred Rates (Tobacco and Non Tobacco)
- No cancer or cardiovascular death of a parent before age 60.
- Blood pressure 150/90 or better.
- Cholesterol under 240 and HDL less than 6.5
- No history of cancer, diabetes, heart disease, etc.
- Maximum 2 moving violations past 3 years.
- No DUI or reckless driving for 5 years.
- Non Tobacco - No use of tobacco products in past 3 years.
- Tobacco - Tobacco use acceptable.
- No history of alcohol or substance abuse.
Standard Rates (Tobacco and Non Tobacco)
- Not more than 1 cancer or cardiovascular death of a parent before age 60.
- Blood pressure 155/95 or better.
- Cholesterol under 280 and HDL less than 8.0
- Some major health conditions allowed.
- Maximum 2 moving violations past 3 years.
- Non Tobacco -No use of tobacco products in past 3 years.
- Tobacco - Tobacco use acceptable.
- No alcohol or substance abuse for 7 years.
